Job Title: Nurse Navigator
Job Description
As a Nurse Navigator, you will be the primary point of contact for patients, guiding them from their first day of treatment. Responsibilities include reviewing treatment processes, managing side effects, providing post-treatment follow-ups, and offering triage and symptom management. You will also facilitate patients' transition from hospital to home, maintain treatment areas, monitor inventory, and coordinate care plans through phone communications.

This is a Contract-to-Hire position with a duration of 6 Month(s).
Work Site
This is a fully on-site position in Schaumburg, Illinois.
Work Environment
PT ROLE - 3x/week on-site (first-shift hours 8am-430pm)
Join a bustling treatment center that serves 120 patients daily. You will work closely with Infusion RNs, other healthcare providers, and CMAs, ensuring comprehensive care delivery. Work hours are three days a week from either 8 AM to 4:30 PM or 8:30 AM to 5 PM with no weekends or holidays.
