Actalent RN Case Manager in Stanford, California

Description:

The RN Case Manager will be floating across multiple department's within the hospital supporting the case management team. Major responsibilities include coordinating all facets of a patient's admission/discharge; performing utilization review activities, including review of patient charts for timeliness of services as well as appropriate utilization of services; and ensuring optimum use of resources, service delivery, and compliance with external review agencies' requirements. In their role, Case Managers, by applying guidelines and collaborating with multidisciplinary teams, influence and direct the delivery and quality of patient care. Familiarity with the EPIC system and InterQual is needed in the role. On top of care coordination it is required to educate patients and families on the current state of care, ensure the care is being reviewed and assessed for quality improvement, and function as a resource to the physicians, nursing staff, and other team members of care.

Additional Skills & Qualifications:

Bachelor's Degree from an accredited college or university Nursing\RN - Registered Nurse - State Licensure And/Or Compact State Licensure required MUST HAVE in hospital experience and care management experience BLS - AHA approved
