Actalent Pharmacist in Ventura, California

A national leader in pharmacy services is looking for a Pharmacist in Ventura County to join their team! With over three decades in long-term care experience, this pharmacy service is looking for someone excited to expand on their skills and grow within their company.

MUST HAVE:

  • 1 year of experience in long term care

  • Pharmacy license

Description:

  • Monday - Friday, 8am - 5pm

  • On site position.

  • Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration.

  • Make recommendations for therapy changes to physicians and facility staff as appropriate.

  • Maintain patient/resident database, including current and past medical history, relevant clinical and laboratory data, and concurrent medications (including over-the-counter and home remedies).

  • Ensure that appropriate laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.

  • Collaborate with the healthcare team to provide clinical and technical support as needed.

  • Document all communication with physicians, nurses, other team members, patients/residents, and their family members in the medical record.

  • Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.

  • Obtain and maintain all required documentation in accordance with federal and state regulatory agencies and KPS policy.

  • Inform the Pharmacy Manager of medication-related errors.

  • Assist facility/agency staff personnel with procedures related to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.

  • Perform other assigned tasks. Conduct job responsibilities in accordance with the Company’s Code of Business Conduct and Ethics, policies and procedures, the Corporate Compliance Agreement, applicable federal and state laws, and professional standards.

Placement: Contract to hire (3 months)
